The VTE risk assessment proformas are NOT interchangeable.

Each proforma should be used on a specific patient group. See below.

Ambulatory lower limb immobilisation

use this risk assessment only for patients seen in the emergency department who are NOT admitted to hospital and are fitted with a lower leg immobilisation such as a cast, boot or splint.

Maternity

Use this VTE risk assessment for all women during pregnancy or 6 weeks post-partum

Day case surgery

Use this VTE risk assessment for all patients having a day case surgical procedure

Paediatric

Use this VTE risk Assessment for all patients admitted onto paediatric wards. Paediatric patients 16 years old and older admitted onto adult wards should be VTE risk assessed using the adult inpatient VTE risk assessment

Inpatient

Use this VTE risk assessment for all medical or surgical inpatients excluding:

  • pregnancy and 6 weeks post partum
  • ambulatory patients seen in the emergency department applied a lower limb immobilisation
  • paediatric patients admitted onto paediatric wards
